Please Note the Following

72 hours before collecting urine sample, avoid consuming fish and shellfish.

48 hours before collecting urine sample, refrain from taking non-essential medications and supplements.

An hour before each urine sample collection, refrain from drinking more than 8 oz of water to ensure enough urine concentration.

Females should not collect samples during menstruation.
Step 1
Take all contents out of the box.
Note: Urine collections are done once upon waking and once around bedtime. Use one card per collection.
Step 2
Make sure to complete the test requisition form and sign it. Without your signature, we cannot perform the testing.
Step 3
Write your name, collection date, and time on the collection card. Otherwise the sample may be rejected by the lab.
Step 4
Open the card up flat, and be careful not to touch the filter paper. To collect your sample, urinate in the clean cup provided and slowly dip the filter paper in the cup, allowing the urine to soak up to the top mark of the paper.
Step 5
Once the filter paper is soaked with the urine sample, tape it or hang it to dry. Make sure the filter paper does not come into contact with anything.
Step 6
Let the filter paper dry completely. This might take 4 to 6 hours. Then, close the top flap.
Repeat steps 1-6 for the second sample collection at bedtime around 9 PM.
Step 7
Place the urine cards in the biohazard bag, seal it, and place it in the refrigerator until you are ready to mail it. Place your sample into the biohazard bag and then into the green smartpac, and seal it. Make sure to write your address. Drop it off at any post box.
Complete Paperwork
Write your name, collection date, and time in the space provided on the collection card. Complete the test requisition form and sign it. Without your signature we cannot perform the testing.
Return
Place your sample into the smartpac and drop it off at any post box for it to be shipped back to us.
